How To Book

Please ask your family doctor / specialist to refer you.

If you do not have a family doctor, please attend a WalkIn clinic to be referred. 

Click on the attached Referral Letter and fill out as best you can and give that to your doctor / Nurse Practitioner - it has all the details on where to forward the form to. 

If you cannot get anyone to refer you, or find that cumbersome to get done, please send us a message on the "Contact" form and we will figure out how to help you.
